Job Description

The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care.

Qualifications for this position include:

1. M-F 8:30-4:30
2. One weekend day shift every 4 weeks
3. One major holiday
4. Willingness to occasionally float to all 3 campuses

Required

• BSN
• 2 years of medical-surgical nursing experience

Preferred

• 1-2 years of acute care, homecare, or population health RN case management experience
• CCM certified
