Job Summary

Case Managers play a critical role in a multi-year, grant-funded program designed to transition disconnected youth and adults with disabilities into competitive, integrated employment through holistic family supports. As part of the support team, the Case Manager works to help families achieve self-sufficiency by connecting individuals and families to community resources, coordinating referrals and applications, and overcoming barriers to employment. Using a wrap-around approach, the Case Manager develops and updates care plans based on ongoing assessments. This grant position is expected to conclude in early 2029.

Primary Duties

Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions. These duties will be evaluated during the Annual Performance Review.

  1. Conduct assessments to identify support needs and set individualized and family goals for self-sufficiency, collaborating with team members such as employment specialists, youth transition specialists, vocational counselors, TANF case managers, and school counselors.
  2. Develop, implement, and update person-centered plans tailored to each participant's needs and changing circumstances.
  3. Connect participants to community resources, assist with service navigation, and guide them through complex systems.
  4. Monitor and address barriers to accessing services, working proactively with participants to facilitate employment and independence.

Additional Responsibilities

  1. Build and maintain relationships with community partners, participants, families, and stakeholders to ensure access to current resources.
  2. Communicate regularly with state vocational rehabilitation counselors.
  3. Stay informed about community programs and services related to healthcare, housing, education, and employment.
  4. Partner with local organizations to ensure effective referrals and up-to-date information.
  5. Record participant progress, referrals, home visits, and interactions.
  6. Participate in team meetings and other collaborative activities.
  7. Identify and provide support resources related to independent living, medical needs, mental health, housing, transportation, and financial planning.
  8. Seek opportunities to enhance knowledge of systems supporting TANF recipients and disconnected youth, participating in community meetings and trainings.
  9. Meet regularly with the Project Director, Project Evaluator, and team members to align efforts with grant objectives.
  10. Perform other duties as assigned.

Qualifications: Education, Experience, and Certifications

  • Bachelor's degree in social work, psychology, or human services required; master's preferred.
  • 2-5 years of experience working with individuals with disabilities, including youth, children, adults, and families, especially with TANF participants, preferred.
  • 2-5 years of experience in case management and developing individualized plans.
  • Compliance with background checks, clearances, and relevant screenings.
  • Valid driver's license and reliable transportation required; good driving record essential.

Knowledge, Skills, and Abilities

  • Proficiency in Microsoft Office.
  • Strong communication and organizational skills.
  • Attention to detail and accuracy.
  • Commitment to social inclusion and strengths-based approaches.
  • Knowledge of care continuum and transition planning.
  • Ability to build positive relationships with participants and their systems.
  • Knowledge of public benefits (SNAP, TANF, Medicaid, Medicare) preferred.

Physical Demands

Must be able to walk, sit, stand, reach, talk, see, and hear regularly. May need to lift or move up to 50 pounds and assist participants with mobility and transfers. Visual abilities include close and distance vision, depth perception, and focus adjustment. Walking stairs is required.

Work Environment

The environment is typically quiet or moderate, with potential for loud noise and outdoor work during various weather conditions.
