Job Summary
Case Managers play a critical role in a multi-year, grant-funded program designed to transition disconnected youth and adults with disabilities into competitive, integrated employment through holistic family supports. As part of the support team, the Case Manager works to help families achieve self-sufficiency by connecting individuals and families to community resources, coordinating referrals and applications, and overcoming barriers to employment. Using a wrap-around approach, the Case Manager develops and updates care plans based on ongoing assessments. This grant position is expected to conclude in early 2029.

Physical Demands
Must be able to walk, sit, stand, reach, talk, see, and hear regularly. May need to lift or move up to 50 pounds and assist participants with mobility and transfers. Visual abilities include close and distance vision, depth perception, and focus adjustment. Walking stairs is required.
Work Environment
The environment is typically quiet or moderate, with potential for loud noise and outdoor work during various weather conditions.
