As we expand across Ohio, we invite a Case Manager to join our collaborative care team, working alongside nurses, nurse practitioners, case managers, and therapists within our integrated agency. KEY RESPONSIBILITIES
QUALIFICATIONS
Education: High School Diploma or GED required. Bachelor’s degree preferred.
Experience: Minimum of 1 year of case management experience.
Physical Effort: Ability to work under stress and irregular hours.
Communication Skills: Excellent verbal and written communication skills are essential.
Interpersonal Skills: Ability to interact professionally and compassionately with colleagues, clients, and families.
Technical Skills: Proficiency in Electronic Health Record (EHR) systems, telehealth, and other relevant software. Fine motor skills required for handling equipment.
Background: Must pass federal and state background checks; educational and work history verified.
