TITLE: Case Manager

RESPONSIBLE TO: Case Manager Supervisor/Regional Leadership

WORK AREA: Community, Clinical work sites

SUMMARY: PATH Behavioral Health, an expanding force in behavioral healthcare, is seeking outstanding individuals to join our dynamic team. We are dedicated to redefining the mental health care experience by offering clients a fully integrated system of care, built on zero barrier access. Our mission is to provide compassionate, high-quality mental health care, especially for those who have been historically underserved. As we expand across Ohio, we invite a Case Manager to join our collaborative care team, working alongside nurses, nurse practitioners, case managers, and therapists within our one-stop agency. Join us in reshaping mental health services and making a difference in the community.

KEY RESPONSIBILITIES:

  1. Act as a liaison between care team members and clients' families.
  2. Arrange family visits, coordinate transportation, and meet clients' needs.
  3. Maintain clear communication between families, referral sources, and PATH Behavioral Health.
  4. Engage in professional case management to support clients' mental and physical health.
  5. Assess clients' needs and develop personalized care plans.
  6. Collaborate with care team members to ensure seamless service delivery.
  7. Monitor clients' progress and adjust care plans as needed.
  8. Advocate for clients to access resources and overcome obstacles to achieve goals.

QUALIFICATIONS:

Education: High School Diploma or GED required; Bachelor’s degree preferred.

Experience: Minimum of 1 year of case management experience.

Physical Effort: Ability to work under stress and irregular hours.

Communication Skills: Excellent verbal and written communication skills, with strong grammar and clarity.

Interpersonal Skills: Ability to interact professionally and compassionately with colleagues, clients, and families.

Technical Skills: Competency in Electronic Health Records (EHR), telehealth, and other relevant software. Fine motor skills for handling equipment and tasks.

Background: Must pass all federal and state background checks; educational and work history verified.
