We are seeking a passionate Case Manager to join their team and support clients in managing their Home Care Packages (HCP).

About the Role

• Develop and manage individualized care plans tailored to clients' needs
• Assist clients in accessing services that promote independence and well-being
• Regularly review and adjust care plans to align with changing circumstances
• Advocate for clients to ensure they receive the best possible care
• Ensure compliance with government regulations and funding guidelines

About You

• Experience in case management, aged care or community services
• Strong understanding of Home Care Packages and aged care legislation
• Excellent communication and problem-solving skills
• A genuine passion for enhancing the lives of older individuals
• Relevant qualifications in social work, aged care, or a related field
• A commitment to delivering values-driven, person-centered care
