We are seeking a passionate Case Manager to join their team and support clients in managing their Home Care Packages (HCP).

About the Role

  • Develop and manage individualized care plans tailored to clients' needs
  • Assist clients in accessing services that promote independence and well-being
  • Regularly review and adjust care plans to align with changing circumstances
  • Advocate for clients to ensure they receive the best possible care
