About the Role
We are seeking a Registered Healthcare Professional (e.g., RN, Social Worker) with Mental Health experience to join the Community Mental Health Team (CMHT). The CMHT provides services via a multidisciplinary team approach, including specialist psychiatric assessment and treatment for adults with moderate to severe psychiatric disorders.
The primary role of the case manager is to support individuals in achieving their personal goals based on their needs. This includes understanding psychiatric disorders and medications, providing community support, and facilitating interpersonal, social, recreational, vocational, and independent living activities.
