Job description

Service Care Solutions is supporting a reputable non-profit provider that is currently recruiting for a Case Manager (also known as a Recovery Worker) to join their team on a permanent basis. This service has supported individuals with addictions for many years and prides itself on being innovative and forward-thinking. They work closely with the recovery communities they serve to provide flexible, responsive services aimed at achieving the best possible recovery outcomes for all service users.

This service is looking for a dedicated individual with experience working with service users with addictions.

The role of a Case Manager

To demonstrate expert and effective practice around substance misuse treatment. Provide advice and brief interventions, some structured key working, assessments, and a needle exchange service.

Key roles and responsibilities for a Case Manager
  • Undertake comprehensive assessments to evaluate the eligibility, suitability, strengths, and needs of service users in relation to the service.
  • Devise, monitor, review, and update recovery care plans to ensure service users’ needs are met in a timely and appropriate manner.
  • Deliver care packages, including psychosocial and psychological interventions where appropriate.
  • Conduct risk/need assessments and implement risk management/care plans and reviews in line with policies and procedures.
Essential requirements for a Case Manager
  • Relevant Degree/L5 qualification (e.g., Social Work, Psychology), OR
  • Diploma in Substance Misuse or equivalent, AND
  • L3 Drug and Alcohol or equivalent.
