By getting involved, you're not just giving aid; you're becoming a part of our mission to build a brighter, more equitable future.?*Who we are looking for:*The Eviction Prevention Case Manager will provide intensive intervention services for people who are at imminent risk of eviction. The case manager will support clients who are at risk of eviction to sustain their tenancy and/or relocate and help mitigate the effects of an unsustainable tenancy. The case manager will assist the tenant to create plans that will help prevent future eviction and homelessness through housing assessment, stabilization, goal planning, and linkages to community services. *Candidates must possess a valid G-class driver?s license and provide a clear driver?s abstract. Candidates must have a vehicle for daily use for client meetings and appointments.*Refer to ACSA Community Services for more information about ACSA?s work.?*About the role:*· Receive and follow up on referrals according to City of Toronto EPIC processes; follow all project processes and requirements· Manage a caseload of up to 20 clients for rapid response eviction prevention and housing stabilization; provide appropriate supports and referrals to services· Provide individualized and flexible case management services· Assess eviction status, analyze issues and develop case plans for eviction prevention· Assess client eligibility for EPIC program and associated program resources; prepare applications for financial resources· Attend Landlord and Tenant Board hearings; provide advocacy as needed· Advocate, mediate and negotiate with housing providers for payment plans and to sustain tenancies· Provide rehousing supports if eviction cannot be prevented· Maintain accurate case management and program records, using SMIS· Prepare statistical and qualitative reporting as required by funder and the organization; meet all reporting timelines and requirements· Attend joint meetings and trainings as set out by the City of Toronto and organization· Work collaboratively with colleagues and participate in team meetings· Attend Case Manager/ Supervisor meetings· Other duties as required.*About you:*· Diploma/Degree in Social Services/Health or minimum of 1 year related experience· Knowledge of the Residential Tenancies Act (RTA)· Strong analytical and problem-solving skills; Excellent interpersonal skills· Demonstrated knowledge of issues of homelessness and community resources· Ability to manage issues tactfully, and relationships with professionalism, and resolve conflicts· Ability to work independently and as part of a team· Knowledge of SMIS is an asset.*Working Conditions*The EPIC Case Manager works from various locations in the community, meeting with clients in housing units and in the clients? buildings, public places and offices. The Case Manager also works from an office at 4155 Sheppard Avenue East and attends meetings and appointments across the City of Toronto as required.*What we offer:*Full time employment with benefits after 6 months3-week vacation, 15 sick days, 2 float days?Salary range
