We are looking for a passionate and dedicated individual to be part of Lewisham’s model of Integrated Health and Social Care, working with multi-disciplinary teams including the voluntary sector, to promote independence and personalisation.
The main purpose of this job is to work as a care management officer to assess the needs of vulnerable adults and their carers.
What’s the role?
Assessing the needs of vulnerable adults and their carers, and supporting them to arrange services to meet those needs. This role is for a part-time Case Management Officer (hours per week).
Assessing and reviewing adult social care plans is a key component of this role, along with accurately recording information on case files.
