Shift: 5x8 Days (Monday–Friday, 8-hour shifts)

Pay: $3,400/week (Gross)

Job Description:

We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager for a 13-week contract assignment in Plymouth, MA. The RN Case Manager will be responsible for coordinating care across the continuum to ensure quality patient outcomes and effective use of resources. This position is ideal for a compassionate nurse with strong clinical and communication skills who enjoys working in a fast-paced, collaborative environment.

Key Responsibilities:

  • Perform comprehensive assessments and develop individualized care plans
  • Coordinate care among interdisciplinary team members, patients, and families
  • Ensure continuity of care through discharge planning and resource coordination
  • Monitor patient progress and modify plans of care as needed
  • Document all case management activities accurately in the medical record
  • Advocate for patient needs, including referrals to community resources or specialists
  • Ensure compliance with healthcare regulations, standards, and best practices

Requirements:

  • Active RN license in the state of Massachusetts (or compact license if applicable)
  • Minimum 3 years of RN Case Management experience preferred
  • Experience with discharge planning and utilization review a plus
  • Excellent communication, critical thinking, and organizational skills
  • Proficient in EMR systems (Epic, Meditech, or similar)
  • BLS certification required
