Position Overview: The Case Counselor will provide case management services, collaborate with clinical staff to coordinate care, educate clients on the continuum of care, and assist with treatment plans.

Specific Responsibilities:

  1. Participate in assessments and development of the service plan.
  2. Implement services according to the individualized service plan (ISP).
  3. Collaborate with the patient and their family or significant others to execute the service plan, explaining available clinical options and their pros and cons.
  4. Complete clinical intake documentation and assessments for risk as per policy.
  5. Document patient progress and maintain comprehensive clinical records, including assessments, services provided, care coordination, and discharge planning.
  6. Work with the clinical team to engage, educate, and coordinate care with patients, families, health providers, and community resources to ensure ISP implementation.
  7. Assist in maintaining, monitoring, and modifying behavioral health services covered under the plan.
  8. Reduce barriers to engagement and meet goals for ISP with collateral contacts.
  9. Ensure referrals for services are made and coordinated with providers and community resources.
  10. Participate in care continuum and discharge planning meetings.
  11. Work towards departmental goals and OKRs.
  12. Perform other duties as assigned.

Education and Experience: High School Diploma and at least one year of professional experience in behavioral health or psychiatry required; a Bachelor's in a social work-related field is preferred. Licensure is not mandatory but a Licensed Drug and Alcohol Counselor (LADC) I or II is highly preferred. Knowledge of healthcare, detoxification, addiction, co-occurring disorders, DSM criteria, and familiarity with Microsoft Office is required.
