A healthcare provider in Cornwall is looking for a Case Coordinator to join their team in Helston. The role involves managing non-urgent referrals, supporting clinical staff, and ensuring timely discharge for patients. Qualified candidates should have good interpersonal and communication skills, as well as a Level 4 qualification in health or social care. An exciting and rewarding opportunity has arisen for a Case Coordinator to join the team in the West Integrated Transfer of Care Hub (iToCH). Currently based in Helston Community Hospital. There would be an expectation for you to hot desk and shadow staff during your induction at Camborne Health Office and Penwith Health Centre. You will play an important role in supporting clinical staff with admission avoidance, arranging community support, case managing non-urgent referrals and facilitating safe and timely discharge from the acute and community hospitals for patients.

You will help provide a seamless service for the people we serve which will include cross boundary and collaborative working with the multi-disciplinary team, along with colleagues in Adult Social Care and other outside agencies in the community setting.
