We are based at the offices of the Bracknell Community Mental Health Team and work as part of a joint team funded by both Health and Social Care. Our team is committed to supporting carers, and your role will involve undertaking carers assessments and providing tailored support to meet their needs.
We work collaboratively and value every team member's input. You will be supported through regular supervision, team meetings and multidisciplinary team (MDT) sessions, with your active participation expected.
