Devon Partnership Trust's Belvedere Ward in Exeter is seeking a Carers Champion and Activities Link Worker. You will work with Carer Link Champions to involve carers in assessment, care planning and discharge, applying the Triangle of Care principles and ensuring carers' voices are valued.

You will also support meaningful therapeutic activities, assist with daily living tasks, and provide high-quality care under supervision, contributing to safer staffing and a patient-centred ward environment.

Formación

• Care Certificate or willingness to complete within probationary period.
• NVQ Level 3 in Health and Social Care or equivalent experience.
• Literacy and numeracy skills appropriate to the role.
• Evidence of ongoing professional development.
• Qualification in dementia care, mental health care or therapeutic activities.
• Training relating to carers' support or family interventions.
• PMVA/physical intervention training.

Responsabilidades

• Promote carer involvement in care planning and reviews.
• Deliver diverse therapeutic activities for patients.
• Provide high-quality, person-centred care under supervision.
• Maintain accurate electronic patient records.
• Attend MDT meetings and handovers.
