At Sunrise, our Care Manager is responsible for providing the highest quality of care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.
RESPONSIBILITIES & QUALIFICATIONS
Responsibilities:
- Build meaningful relationships with residents and their families, assisting with activities of daily living, attending to individual care needs, and understanding their unique preferences and personalities.
- Notify management of changes in residents' conditions and recommend adjustments in care and services.
- Assist residents in life skills and other enriching activities as indicated in their profiles.
- Incorporate multi-sensory experiences into residents' daily routines.
- Participate in developing and updating Individualized Service Plans (ISP).
- Manage a designated group of residents during the shift, ensuring their well-being through regular checks.
- Collaborate with the community team to ensure compliance with OSHA and promote safety and risk management policies.
- Respond to dining needs of residents and guests, maintaining Sunrise hospitality standards.
- Encourage and assist residents in life skills and activities as per their ISP and profiles.
- Uphold our Principles of Service in serving residents and guests.
Qualifications:
- Dedication and passion for serving seniors, with excellent customer service skills.
- Positive attitude, flexibility, and teamwork skills.
- High School diploma or GED; additional certifications may be required by state regulations.
- Must be at least 18 years old.
- Previous experience with seniors preferred.
- Ability to make decisions in the best interest of residents.
- Effective written and verbal communication skills.
- Strong organizational and time management skills.
- Good judgment, problem-solving, and decision-making abilities.
