Job Opportunity: Care Transfer Hub Manager

You will be hosted by Sheffield Teaching Hospitals FT and work on behalf of Sheffield Place for the people of Sheffield. This is a system post collaborating with partners from the Sheffield Integrated Care Board, Sheffield Teaching Hospitals NHS Trust, and Sheffield City Council.

You will lead a multidisciplinary team including RNs, AHPs, administrative staff, voluntary sector colleagues, and adult social care professionals, all focused on supporting people to return home after discharge. The Care Transfer Hub emphasizes 'Home First' — reducing harm and improving outcomes by facilitating timely and appropriate discharges.

The Care Transfer Hub Manager will provide operational leadership and coordination to enable transfers of care from acute and community health beds, supporting recovery in the community. You should be a credible leader with a strong understanding of hospital discharge processes, a strengths-based mindset, and an analytical, evidence-based approach.

You will be confident in leading change, managing uncertainty, and exemplifying visible leadership. Your focus will be on improving patient outcomes by ensuring discharge processes are efficient, timely, and appropriate, including expanding our digital discharge tools and data sharing across health and social care sectors.

Building on our current initiatives, you will scale and embed the discharge transfer model to enhance coordination across health and social care, applying lessons learned to mental health discharges.
