The Care Transfer Hub Clinical Lead will provide clinical and professional leadership for clinicians and service leads across the Community Care Group, specifically within the Care Transfer Hubs at RDE Wonford and North Devon District Hospital.

The post holder will have senior clinical oversight of the hospital discharge pathways (from both acute sites) and will support the teams to ensure efficiency and effectiveness of assessment and interventions to support hospital discharge. They will work with colleagues within the Care Group (in Community on the Intermediate Care Pathways) and across the organisation (e.g. with ward clinicians, site practitioners etc) with a focus on improving patient flow through the Care Transfer Hubs.

Key Responsibilities:

  • Provide visible and accessible leadership for clinical teams, operational colleagues, patients, and service users.
  • Represent the Care Group at Trust-wide and System level meetings.
  • Continually review and improve the quality and efficacy of clinical practice and care.
  • Offer professional advice and support in complex clinical situations interpreting Trust and national guidance
  • Act as a positive role model, upholding Trust values and professional behaviour.

The ideal candidate for the Care Transfer Hub Clinical Lead role will possess extensive clinical experience within both hospital and community settings. They will have demonstrated their ability to lead and implement change effectively at both team and service levels. Skilled in managing their own time and competing priorities, the candidate will be adept at synthesising information from various sources and translating it into professional guidance.

Experience working across organizational boundaries is essential, as is the ability to lead successful projects, audits, and service changes. The candidate must exhibit excellent communication and interpersonal skills, including the ability to resolve conflicts and negotiate effectively. A thorough understanding of relevant professional codes of conduct and standards is required.
