We are on a journey of developing our complex care offering to support children, young people, and adults in their own home environments who have complex conditions and live in the area. Our Care Supervisors have an allocated caseload of clients and associated staff teams. They engage closely with our clients, family members, and professionals, monitoring, supporting, and developing a dedicated staff team. They guide, motivate, and inspire the team to focus on achieving the clients’ goals and outcomes.

About the Role

Our Care Supervisor role is community-based. The successful individual must be organized, possess strong planning skills, and be able to operate independently as well as part of our well-established team. They are expected to be highly visible in the community, visiting their assigned clients regularly and being a pivotal part of the service we provide. The core of the role is to inspire a team of Support Workers to help our clients reach their potential.

Our service includes weekends and overnight support, so flexibility is essential to ensure the service is monitored through staff spot checks, reviews, and observations.

Additionally, some clients require support at school and engagement during the day. Case Leaders plan their own schedules to meet service needs while maintaining a good work-life balance.

Our Case Leader will participate in an ‘on-call’ rota, which is the service’s emergency out-of-hours facility, on a 1 week on, 1 week off basis.

The responsibilities include:

  1. Producing and maintaining person-centred care plans for all new clients, ensuring risks are assessed and recorded appropriately.
  2. Supporting and developing staff in designated care packages through close supervision and monitoring to ensure consistent care standards.
  3. Engaging with and supporting staff through team meetings, supervisions, spot checks, annual reviews, and observations, including covert and night-time checks.
  4. Conducting regular reviews with clients, involving family members, professionals, and advocates where appropriate.
  5. Monitoring, auditing, and maintaining records within clients’ homes.
  6. Introducing new staff to clients, ensuring they are aware of individual care plans and focus on clients' goals and aspirations.
  7. Recognizing positive feedback from clients, families, and professionals, and sharing this with staff through various channels.
  8. Conducting live and dynamic audits on medication and record-keeping to ensure best practices.

Minimum requirements include:

  • At least 3 years of community complex care experience.
  • Level 4 Diploma or higher in Health and Social Care.
  • Experience in leading or managing a team.
  • A full UK driving license and own vehicle.
