Barrack Lane Medical Centre is seeking a care navigator for a full-time position in Ipswich. This role is vital as it involves direct patient interactions, requiring excellent communication and a caring approach. Ideal candidates will be team players able to maintain professionalism under pressure.
Barrack Lane Medical Centre has an opportunity for a care navigator to join our friendly, supportive and professional team on a full-time basis, 37.5 hours per week. Although you will be employed by the Barrack Lane and Ivry Street PCN you will be based at Barrack Lane.
The care navigator is a very important role within the surgery as they are the first person a patient speaks to or sees when they contact the surgery.
General practice is a challenging environment that requires a calm and caring approach with the ability to work under pressure. The ideal candidate will be a highly motivated team player who has customer service/patient experience and offers excellent communication skills.
Barrack Lane is a busy practice in the town centre of Ipswich, Suffolk. We have a practice population of over 20,000 patients. The practice is based on two sites, the main surgery in the centre of the town, and a branch surgery at Stoke Park.
Barrack Lane is part of a PCN (Primary Care Network) with Ivry Street Medical Centre, in Ipswich, and the two practices cover a population of 32,000 patients. The PCN allows the practices to share resources and offer an enhanced service to our patients. The PCN employs care co-ordinators and social prescribers to work with patients, especially our vulnerable patients, to help them to access the best care.
