Overview

Job Summary

Kenyon Medical Centres has an opportunity for a Care Navigator to join our team on a part-time basis for 21.5 hours a week. The Practice serves 13,900 patients with 3 GP partners and 3 FSP, along with GP Trainees/Students and ARRS.

The Role

Your role as a Care Navigator is to be the face of the Practice. You will work in a fast-paced environment, handling reception duties, answering telephones, dealing with queries and performing administrative tasks.

Hours

Monday: 14:00–18:30

Tuesday: 14:00–18:30

Wednesday: 14:00–18:30

Thursday: 14:30–18:30

Friday: 14:30–18:30 (21.5 hours a week)

Saturday: On a rota basis (approximately twice a year).

Main duties
  • Greeting and directing all patients
  • Making appointments
  • Answering queries and complaints from patients and outside agencies, signposting to appropriate staff
  • Answering telephone calls
  • Accepting, issuing and handing out prescriptions, letters, forms, specimen bottles, etc.
  • Being aware of protocols and policies and advising patients of practice procedures
  • Organising ambulance transport for patients
  • Handling enquiries accurately and avoiding dispensing advice or breaches of patient confidentiality
  • Registering new patients
  • Accurately maintaining patient records, photocopying, filing and scanning
  • Kenyon's clinical system is EMIS Web

Job Description and Responsibilities

Duties and Responsibilities

Telephone Duties

  • Dealing with appointment and administration requests using AccuRx templates
  • Signposting patients to NHS and other providers
  • Giving out test results in line with surgery policy
  • Dealing with general enquiries by telephone and face to face
  • Taking and recording clinical messages
  • Instructing patients to call 999 when emergency protocol applies

Care Navigator Duties

  • Welcome and direct patients and visitors; ensure GDPR-compliant visitor paperwork
  • Make appointments per clinical protocols
  • Process prescriptions and check medication/request validity
  • Explain outstanding queries and hand over to next shift as needed
  • Assist the Triage Team, Doctors and Nurses during surgery
  • Collect payments for NHS and non-NHS services
  • Handle general enquiries and complaints via telephone, email, text and in person per SOP
  • General housekeeping and maintain notice boards
  • Observe health, safety and infection control guidelines
  • Act as a Chaperone when needed (training provided)

Prescriptions

  • Handle repeat prescription requests per dispensing protocol
  • Record and store medication deliveries per SOP

Administration

  • Input and extract patient information in EMIS Web and other in-house systems
  • Keep mandatory training up to date
  • Manage address changes, scanning into patient notes, patient online services, and temporary residents
  • Open, scan and code incoming post using Docman
  • Support admin with PCSE medical records and register patients on EMIS; scan paperwork for new patients
  • Work at both Kenyon Medical Centre sites front of house
  • Any other duties commensurate with the grade
Person Specification

Essential

  • Educated to GCSE level or equivalent
  • Experience of working with the general public, reception work, and healthcare setting
  • Excellent written and oral communication skills
  • IT skills; competent in Office, Outlook, and Excel
  • Polite telephone manner; able to work to deadlines and under pressure
  • Interpersonal and problem-solving skills; ability to work as part of a team and autonomously
  • Knowledge of EMIS Web; flexibility to work outside core hours
  • Disclosure and Barring Service (DBS) check
  • Appropriate car insurance/driving licence
  • Previous work references

Desirable

  • Active signposting or Care Navigator training
  • Healthcare qualification (level 2) or equivalent; customer service qualification (NVQ) or equivalent
