A local GP practice in Sittingbourne seeks a Care Navigator to maintain appointment systems and assist patients. The ideal candidate will have GCSEs in English and Maths, customer service experience, and familiarity with medical software. Flexibility and teamwork are essential skills for this full-time or part-time position offering diverse responsibilities.
Applications are invited for the post of full time or part time Care Navigator for our busy GP Practice located in the heart of Sittingbourne town centre.
Working hours are 37.5 hours per week, Monday to Friday, on a split shift rota system. Some evenings and early mornings are required.
The successful applicant must be flexible, be able to work under pressure and enjoy working within a team. A good standard of education and previous experience of working in a GP surgery or in a customer facing role are essential.
Duties can include but are not limited to, greeting and directing patients, effective use of the appointment system, booking appointments, processing of information and assisting patients as required. Answering the telephone to patients and outside agencies.
To act as the central point of contact for patients, the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community services.
