Care Navigators, use a coaching framework, to manage wellbeing in the pre and post release period, to prevent a return to ill health as well as a reduction in reoffending. By preventing this decline in health, through robust reconnection with health services in the community we will reduce health inequalities for those leaving the criminal justice system and provide support to allow a seamless, and effective transition to independent community living.
