A local healthcare provider in Crewe is seeking a Care Navigator to join their busy GP surgery. The role includes greeting patients, managing appointments, and supporting a multidisciplinary team. Ideal candidates will have excellent communication skills, the ability to work both independently and as part of a team, and experience in public-facing roles.

Responsibilities

  • Greeting and directing patients at the practice.

We are large, busy GP surgery in the Crewe area. We have an exciting opportunity for the right candidate to join our team as a Care Navigator.

Main duties of the job

Meeting and Greeting patients, arranging appointments and dealing with general administration for the practice.

Job responsibilities

Care Navigators are responsible for undertaking a wide range of reception dutiesand the provision of general support to the multidisciplinary team. Duties can include but are not limited to,greeting and directing patients, effective use of the appointment system,booking appointments, processing of information and assisting patients asrequired. To act as the central point ofcontact for patients, the distribution of information, messages and enquiriesfor the clinical team, liaising with multidisciplinary team members andexternal agencies such as secondary care and community service providers.
