We currently have an opportunity for an experienced Care Manager. The Care Manager will coordinate concurrent utilization management activities house-wide and is the main link between the patient, health care team, organization, and payors during the patient's hospital stay.

Responsibilities include:

  1. Coordinate with the appropriate manager(s) on a daily basis to assess the inpatient census for appropriate alternative health care service needs.
  2. Utilizes pre-approved criteria and guidelines to validate medical necessity of continued stay and appropriateness of treatment and discharge planning.
  3. Predicts and plans for patient's needs from pre-admission, through acute and sub-acute care and post-discharge, in collaboration with the member and providers.
  4. Monitors and facilitates appropriate utilization of resources using evidenced-based clinical criteria.
  5. Participates in an interdisciplinary health care team to achieve positive member outcomes.
  6. Functions as a resource to the clinical team regarding approved criteria, practice guidelines and alternative treatment options.

In order to help us continue to provide exceptional care, we're looking for you to have these skills:
  1. 1 or more years of case management/ discharge planning experience.
  2. An active NH/Maine/Compact RN License or an active Maine Social Worker License.
  3. BLS Certification.
