Do you have experience as a Care Manager?

Active Care Group's Care in the Home division continues to grow, providing safe, reliable, and outcome-based care to clients through our exceptional support workers, clinicians, and management team.

We have an exciting opportunity for an experienced Care Manager with NVQ 4 or above in Health and Social Care to join our friendly and supportive team. This role covers care packages in North England, approximately from Sheffield to Yorkshire. It is highly rewarding, as our care enables clients to remain in their homes and maintain independence as much as possible.

Active Care Group has over 25 years of experience delivering high-quality complex care tailored to individual needs, preferences, and lifestyles, considering not just clinical needs but also personal, social, and emotional aspects. Our care extends to the wider family and social circle, recognizing the broader impact of complex conditions.

What you'll be working:
  • Work from home with visits to clients
  • 37.5 hours per week, mainly Monday-Friday 9am-5.30pm, with flexibility as needed
  • Willingness to participate in an on-call rota during nights and weekends
What you'll be doing:
  • Owning care packages in your area
  • Collaborating with the Business Development Manager to review referrals
  • Producing assessment reports for care packages
  • Building strong relationships with clients, families, and external bodies
  • Ensuring high-quality, client-focused care
  • Supporting recruitment and training of Support Workers
  • Maintaining accurate and timely records
What you'll have:
  • NVQ 4 or above in Health and Social Care
  • Passion for delivering high-quality, client-focused care
  • Experience at Team Leader level or higher in care
  • Previous management experience is advantageous
  • Excellent verbal and written communication skills
  • Positive attitude and commitment to continuous improvement
