Overview

Job Scope: We are looking to recruit a Care Manager to lead and develop complex care services within a community setting. As a Care Manager, you will oversee a caseload of clients with complex health needs, ensuring that every care package is safe, person-centred, and responsive to changing requirements.

The Care Manager will take ownership of transitions, start-ups, and the ongoing management of care packages, working closely with families and multi-disciplinary teams to deliver the highest standards of support. You will regularly review care plans and risk assessments, maintaining full compliance with safeguarding and regulatory standards.

Responsibilities
  • Lead and develop complex care services within a community setting; oversee a caseload of clients with complex health needs to ensure safe, person-centred, and responsive care packages.
  • Take ownership of transitions, start-ups, and ongoing management of care packages; work closely with families and multi-disciplinary teams to deliver high standards of support.
  • Regularly review care plans and risk assessments; maintain full compliance with safeguarding and regulatory standards.
  • Provide leadership, mentoring, and development opportunities for care teams; ensure staff are trained and confident to deliver exceptional care in the community.
Requirements
  • Experience in managing complex care packages, including conditions such as spinal injuries, acquired brain injuries, motor neuron disease, or multiple sclerosis
  • Strong knowledge of CQC regulations, compliance, and safeguarding practices
  • Proven leadership skills with the ability to motivate and mentor care teams
  • Excellent organisational and time management abilities
  • A people-first approach with passion for delivering exceptional care
  • A full driving licence and access to your own car
