Key Role Responsibilities
  • Conduct thorough assessments of residents’ health and medical needs.
  • Develop, implement, and evaluate individualized care plans in collaboration with the healthcare team. Administer medications and treatments as prescribed, ensuring compliance with safety standards.
  • Monitor residents' vital signs and report any changes in health status to the appropriate healthcare professionals.
  • Provide support and education to residents and their families regarding health management and care plans.
  • Assist with daily living activities as needed, promoting resident independence and dignity.
  • Maintain accurate and timely documentation of care provided and resident progress.
  • Participate in staff meetings and training sessions to promote continuous improvement in care delivery.
