Job description

NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.

This is a permanent, fulltime role of 37 hours per week, with a shift pattern of Monday to Friday.

Position Overview

The West Dumbarton Older Adult CMHT in Goldenhill Resource Centre, Clydebank is looking to recruit a Band 6 Care Home Liaison Nurse to their team.

Responsibilities
  1. Provide mental health nursing advice, support, and education to identified care homes within West Dunbartonshire HSCP.
  2. Work closely with Multi-Disciplinary colleagues within the Older Adults’ service.
  3. Assess the mental health needs of clients referred from the care homes.
  4. Liaise with local community services and work closely with the clients' RMO (GP) to agree and co-ordinate care packages.
  5. Provide assessment and diagnostic services to the care homes, ensuring ongoing care management.
  6. Regularly review care plans to maintain clients’ levels of ability and prevent hospital admission.
  7. Link with the hospital discharge service to facilitate earlier discharges following the identified care pathway.
  8. Manage own caseload and ensure effective communication between care homes and the local Community Older Adult Service.
