Job description

A Supported Living & a Domiciliary Care Company is looking for Care Coordinators to join their team at Kingston, South West London. In this role you will be required to be effectively and efficiently managing the delivery of high-quality care services to our clients.

Duties
• To accept, carry out assessments for new client referrals and ensure client schedules are matched to their needs and requirements.
• To work closely with the recruitment team to ensure sufficient staffing levels are achieved.
• To add and maintain all client and care professional information onto the electronic scheduling system and the company software.
• Create, update and audit client care plans and assist with digital care planning.
• Maintaining accurate client and care professional records on company software
• Carry out reviews and action these accordingly
• Work in line with CQC standards and regulations
• Carry out any other duties deemed necessary for the successful operation of the business.
• Providing personal care to clients during periods of staff absence.

Skills / Qualifications:
• Level 3 NVQ in Health and Social Care or equivalent or working towards the qualification
• Experience in the care sector delivering a wide range of personal care services is essential
• Knowledge and understanding of Legislation and Regulations specific to Health and Social Care
• Experience of working in a scheduling role within a Domiciliary care and other relevant environment
• Good working knowledge of IT systems with experience of Microsoft Office 365
• The ability to learn and adopt new technologies where appropriate
• Excellent organisational skills with the ability to work on own and under pressure
