Care Coordinator - The Vale of Evesham PCN

Abbey Medical Practice, part of the Vale of Evesham PrimaryCare Network (PCN), is looking for a dedicated and compassionate CareCoordinator to join our team. We are committed to providing coordinated andperson-centred care, especially for patients with complex needs and those incare homes.

Main duties of the job
  • Develop personalised care plans in collaboration with patients, carers, and healthcare professionals.
  • Help patients manage appointments, access services, and understand their care options.
  • Coordinate with local health and social care providers to deliver comprehensive support.
  • Identify patients who would benefit from additional services, such as Social Prescribers or Lifestyle Advisors.
  • Maintain accurate and up-to-date records of care packages and follow up on referrals.

Job responsibilities

As a Care Coordinator, you will play a key role insupporting patients and their carers, ensuring they have access to the rightservices and care.
