Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients who are discharged from hospital into Home First will receive step-down intermediate care, involving community-based assessments and interventions provided in their own homes to support recovery and monitor their health in the early days post-discharge to prevent re-admission.

You could become part of our exciting service, bringing the benefits of a wraparound recovery service from hospital discharge. The principle of the service ensures that recovery involves working towards the person being as independent as possible through enablement.

The focus of this role is providing prompt and efficient coordination and administrative support to a multidisciplinary team (MDT).

You will be the point of contact for service users, their families, and carers when information on crisis and short-term care support is required. This includes enquiries about visits, providing updates, and listening to concerns.

A key duty will be processing and administering referrals using criteria guidelines and seeking advice from team members when needed. You will signpost referrals to MDT members and other services as appropriate while monitoring, tracking, and reporting on progress within the patient pathway. Any issues or delays should be escalated to the Operational Team Lead for appropriate action.
