Do you want to be part of a dynamic and evolving service? Join us at this exciting time in our new and evolving Home First Teams and make a difference every day!
Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients who are discharged from hospital into Home First will receive step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitor their health in the early days post-discharge to prevent re-admission.
You could become part of our exciting service, bringing the benefits of a wraparound recovery service from hospital discharge. The principle of the service ensures that recovery involves working towards the person being as independent as possible through enablement.
We are looking for a Care Coordinator to join us. If you’re organized, proactive, and seeking a dynamic role in a team that provides cutting-edge, excellent community care, we’d love to hear from you!
Note: Due to recent changes in UK immigration and visa rules, this role is unlikely to be eligible for Skilled Worker visa sponsorship. Applicants without the right to work in the UK may not be considered. Candidates requiring sponsorship should review eligibility criteria on the gov.uk website before applying.
The role focuses on providing prompt and efficient coordination and administrative support to a multidisciplinary team (MDT).
You will be the point of contact for service users, their families, and carers when information on crisis and short-term care support is required. This includes enquiries about visits, providing updates, and listening to concerns.
A key duty will be processing and administering referrals using criteria guidelines and seeking advice from team members when needed. You will signpost referrals to MDT members and other services as appropriate, while monitoring, tracking, and reporting on progress within the patient pathway. Any issues or delays should be escalated to the Operational Team Lead for appropriate action.
