A healthcare provider in the UK is seeking a Care Coordinator to support a multidisciplinary team in delivering community-based care. Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients discharged from hospital into Home First receive step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitor health post-discharge to prevent re-admission. Home First also provides acute reablement for up to 10 days, supporting individuals to achieve functional goals and work towards independence in daily activities.

The role focuses on providing prompt and efficient coordination and administrative support to a multidisciplinary team (MDT).
You will be the point of contact for service users, families, and carers regarding their crisis and short-term care support. This includes inquiries about visits, providing updates, and listening to concerns.
A key duty involves processing and administering referrals using established criteria, seeking advice from team members as needed. You will signpost referrals to MDT members and other services, monitor and report on progress within the patient pathway, and escalate issues or delays to the Operational Team Lead for appropriate action.
