Opportunities for experienced Care Coordinator with a proven track record in delivering high quality, person-centred care and support to service users with diverse needs. Experience in a similar role within the health and social care sector, preferably in supported living services, with a Level 3 (or higher) qualification in Health & Social Care or equivalent. The ability to demonstrate a strong understanding of CQC regulations, safeguarding, and person-centred care.
The ideal candidate will have excellent organisational, communication, and leadership skills, and the ability to work flexibly, including occasional evenings and weekends. Proficiency in IT and Electronic Care management systems is essential.
Knowledge of Positive Behaviour Support (PBS), Trauma-Informed Care, Mental Health, Learning Disabilities, and behaviours that challenge would be advantageous.
