We are looking for a highly organized and dedicated Care Coordinator to join our St Helens branch. This is a vital role in ensuring the smooth and efficient delivery of care services, supporting both our staff and Service Users.

Your Role & Responsibilities:
  1. Scheduling & Coordination – Organize and manage rotas to ensure Service Users receive timely, high-quality care
  2. Client Assessments – Assist in conducting assessments of tasks and risks to create personalized care plans
  3. Supporting Care Staff – Provide ongoing guidance and support to care workers, ensuring high standards are met
  4. Liaising with Stakeholders – Communicate with Service Users, families, and healthcare professionals to ensure seamless care delivery
  5. Problem Solving – Address and resolve any scheduling issues, staff absences, or last-minute changes efficiently
  6. On-Call Support – Participate in the out-of-hours rota (1 in 4 weekends & 1 night per weekday) to provide emergency support

What We’re Looking For:
  1. Experience in care coordination or a similar role (preferred)

  5. Full UK driving license & access to a vehicle at all times
  6. Ability to work under pressure in a fast-paced environment
