Job Description

Role: Care Coordinator – Social Worker

Band: 6

Working Hours: Monday – Friday, 9am – 5pm

Responsibilities
  • Conduct FEP Assessments and provide early interventions in psychosis as a care coordinator.
  • Assess and provide care under CPA for individuals experiencing FEP.
  • Demonstrate understanding of the Early Intervention in Psychosis model principles. RTT experience is essential.
Skills
  • Prompt FEP Assessments and strong assessment skills.
  • Excellent communication, report writing, and feedback to MDT.
  • Sound Knowledge of the Mental Health Act and current mental health treatment pathways.
  • Experience with EIP is essential.
Qualifications & Requirements
  • At least 6 months healthcare experience.
  • Right to work in the UK.
  • References covering the last 3 years of clinical employment.
  • NMC Registration.
  • Overseas Police Check, if applicable.
  • Indemnity Insurance.
  • Up-to-date CV.
