Care Coordinator - Shoreham, Petts Wood & Longfield, Shoreham-by-Sea

This role involves scheduling and coordinating care services for clients, building relationships with clients and care professionals, and ensuring efficient service delivery.

Responsibilities:
  • Develop effective schedules considering travel, holidays, training, and cancellations.
  • Respond to schedule changes and liaise with team members.
  • Match Care Professionals to clients and facilitate introductions.
  • Maintain consistency in care schedules and foster positive relationships.
  • Collaborate with recruitment and care teams to meet staffing needs.
  • Manage client and Care Professional information in the electronic system.
  • Conduct care consultations, risk assessments, and develop care plans.
  • Perform quality assurance visits and participate in on-call duties.
  • Ensure compliance with company policies on equality and diversity.
  • Willingness to carry out emergency care calls and drive as needed.
Qualifications:
  • Experience in scheduling within home care or logistics.
