Care Coordinator/Scheduler (CNA required)

Join to apply for the Care Coordinator/Scheduler (CNA required) role at Benchmark Senior Living.

We are seeking a compassionate Resident Care Coordinator to join our team. The Resident Care Coordinator assists the Resident Care Director and Memory Care Director in managing the resident care department, including hiring, training, scheduling, and disciplinary actions. This role also ensures departmental compliance and is part of the community management team.

Responsibilities
  • Ensure compliance with all resident care and state regulations (e.g., medication management, service plans).
  • Monitor overtime usage.
  • Schedule resident care staff for Assisted Living and Memory Care.
  • Assist in developing and implementing care and service plans based on residents' needs.
  • Assess, communicate, and document residents’ status accurately.
  • Communicate staffing needs, including Lead Aide and Medication Aide requirements.
  • Observe and report significant changes in residents’ behavior and health.
  • Follow up on incidents and accidents, and communicate with residents and families.
  • Co-facilitate RCA meetings.
Requirements
  • Must be a Certified Nursing Assistant or Home Health Aide with valid Massachusetts certification.
  • Minimum of two years’ experience working with elders, with supervisory and management skills.
  • Valid driver’s license.
  • Physically capable of moving at least 50 lbs. without assistance.
