Job description

Plymouth City Council Independence at Home Management Team is pleased to announce a rare opportunity for a permanent Reablement Care Coordinator [Grade E] FTE SCP 15 -20. The position offers a starting salary of £29,093.00 to £31,586.00 for the year 2024/25, with annual incremental rises.

The salary is based on a 30-hour contract, with the successful candidate working on a rotation over a three-week period with an established team of coordinators:

  1. Week 1: Monday, Friday 16:30-23:00, and Saturday and Sunday 14:45-23:00
  2. Week 2: Tuesday, 16:30-23:00, Saturday and Sunday 06:45-14:45
  3. Week 3: Wednesday and Thursday 16:30-23:00

The role includes annualised bank hours and offers some home working in the evenings.

We seek a dynamic, sensitive, and enthusiastic individual to join our professional care team. This role provides an excellent opportunity to support individuals with diverse needs to maintain or enhance their independence at home and contribute to their wellbeing.

Full training and support are provided. Candidates should possess:

  • Effective verbal and written communication skills to build rapport and record health and wellbeing information
  • Strong time management and organisational skills
  • IT and scheduling software knowledge
  • An understanding of dignity, respect, and sensitivity when working with vulnerable and older people
  • The ability to motivate and work with service users, including those who may be challenging, disengaged, or terminally ill

Additional essential requirements include:

  • GCSE (A-C) or equivalent in English and Maths
  • Health and Social Care Diploma Level 2
  • Full Driving Licence and access to a vehicle for work purposes
  • A commitment to continuous professional development
