Weare looking for an organised and patient-focused Care Coordinator (PrescribingSupport) to join our Primary Care Network (PCN) team as part of the AdditionalRoles Reimbursement Scheme (ARRS). This is a non-clinical role focused onimproving medication-related support for patients with complex needs, helpingto streamline prescription processes and enhance personalised care planning.
Youwill work closely with clinical staff, including pharmacists and GPs, andsupport patients in navigating prescription services, especially those managinglong-term conditions or taking multiple medications. This is not a pharmacytechnician post but one that complements clinical roles through effectivecoordination and communication.

This vital role bridges patient-facing services and prescription administration, directly supporting GPs and Clinical Pharmacists to streamline medication processes and improve patient care.

This role may require working across all five practices within the Primary Care Network.
