The Care Coordinator- Onsite plays a vital role in optimizing patients’ recovery. They conduct weekly functional assessments and collaborate with interdisciplinary teams to coordinate discharge planning, ensuring smooth transitions and informed decisions with patients and families. The role acts as a link between patients and healthcare providers to facilitate efficient care transitions.
