Overview

JustOT are looking to recruit a band 6 Community Care Coordinator Occupational Therapist for a locum position with our client in London. The pay for this position is £22-£26 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.

Responsibilities
  • Provide specialist nursing care for individuals with learning disabilities and mental health needs through clinical assessment and a stepped-care model within a MDT.
  • Act as care coordinator, manage access to services, maintain accurate records, and ensure safe, effective practice in line with NMC standards.
  • Collaborate with service users, families, carers, and professionals to deliver person-centred, socially inclusive care for those with complex or challenging needs.
Qualifications
  • Registered, RMN, RNLD or HCPC
  • Degree-level education
  • Experience working within a community mental health setting
  • Post-qualification experience working with adults with learning disabilities and mental health needs, or strong evidence of transferable skills
  • Strong clinical knowledge in supporting clients with complex needs and challenging behaviours
  • Skilled in risk assessment and care planning
  • Confidence participating in MDT meetings and contributing to clinical discussions
  • Successful candidates must hold a degree in occupational therapy and be HCPC registered.

  • Experience working in partnership with families and carers
  • Car owner
