A healthcare organization is seeking a proactive Care Coordinator in Royal Tunbridge Wells. The role involves supporting patient access to service and enhancing practices in collaboration with a clinical team. West Kent Primary Care is looking to recruit a Care Coordinator to support Mote & Wallis Primary Care Network.
This role will support The MoteMedical Practice and Wallis Avenue Surgery in expanding capacity for patientsto access a clinical professional. The practices have a combined patientpopulation of around 17,000. The post holder will work across three sitesalongside other PCN memebrs of staff. The PCN alsoprovides MSK and Childrens Social Prescribing services.
The post holder will mainly be based at Staplehurst Health Centre, with some requirement to travel to Mote Medical Practice and Wallis Avenue Surgery.

Mote andWallis PCN would like to recruit a proactive and enthusiastic Care Coordinatorto support their practices in expanding services available to their patients. This role will require the post holder to support practices withachieving Quality Outcomes Framework (QOF), Investment and Impact Fund (IIF)Direct Enhanced Service (DES) requirements; full support and training will begiven. In addition, you will be working closely with the clinical team toorganise and monitor patient access to services in particular chronic diseaseclinics.
