Salary: £28,000 - £30,000 per annum

About The Role

As a Care Coordinator, you will play a pivotal role in ensuring the seamless delivery of care to our clients across a defined geographical area. You will be responsible for coordinating staff rotas, maintaining continuity of care, and building strong relationships with carers and clients. You’ll match the right support workers to each client based on their needs, personalities, and interests—ensuring every care package is not only compliant, but compassionate and person-centred.

This position offers a blend of office-based and remote working, giving you flexibility while remaining connected to your team.

Key Responsibilities

  • Coordinate day-to-day care delivery, ensuring timely and consistent service provision.
  • Build and manage weekly rotas, addressing any gaps or last-minute changes effectively.
  • Match carers to clients based on compatibility, skillset, and shared interests.
  • Respond swiftly to service issues and changes in care requirements.
  • Supervise and support care staff, providing guidance and promoting a culture of excellence.
  • Maintain strong communication with clients, families, and professionals.
  • Participate in the out-of-hours On Call rota as part of a shared team responsibility.

What We're Looking For

  • Minimum 1 year of experience as a Care Coordinator in the Health & Social Care sector.
  • Proven ability to work under pressure, with a calm and solution-focused mindset.
  • Highly organised, with the ability to manage competing priorities effectively.
  • Excellent written and verbal communication skills.
  • Strong IT proficiency, including Microsoft Office and electronic rostering systems.
  • A natural relationship-builder who thrives on creating positive outcomes for others.
  • A genuine passion for delivering high-quality, client-centred care.
