Job summary

CCH Group in Worcester is seeking a Care Coordinator to organise and deliver high‑quality home care in the community. You will manage referrals, create rotas for care workers, and liaise with professionals to monitor service users' conditions.

This role requires excellent organisation and previous healthcare experience, with a solid understanding of CQC standards, care plans, and rotas. NVQ Level 3 is desirable; a driving licence is required; hours are Monday to Friday 9am–5pm with on-call
