NHS is seeking a Care Coordinator to join its Early Intervention Team in Bury for a 6-month fixed-term contract. The role involves managing a caseload for individuals with first episode psychosis and requires a relevant degree along with experience in mental health settings. We are looking to recruit a Mental Health Practitioner /Care Coordinator to work in theBuryEarly Intervention Team.
The Early Intervention team are a multi disciplinary team with staff from a range of professions including, mental health nurses, CBT therapists and support workers. The team has a dedicated Full Time Consultant Psychiatrist.
You would be responsible for a defined caseload of individuals experiencing first episode psychosis as a care coordinator / mental health keyworker within the team, as such you would work with individuals to meet their identified needs using outcome measures, a recovery approach and facilitate discharge from secondary mental health services, or referral on as appropriate . Additionally the post holder would contribute to duty and clinic work and participate in team development issues.
