We are seeking a dynamic, compassionate, and detail-oriented Care Coordinator/Deputy Care Manager to join our dedicated team and help us make a real difference in the lives of people we care for across the Wirral.

Role

As a Care Coordinator/Deputy Care Manager, you will work alongside the Care Manager to oversee the delivery of person-centred care services for our clients and develop our client base. You will coordinate needs assessment and care plans, build a new service from the ground up and manage a team of carers, and ensure the highest standards of care are consistently met. Responsibilities
  • Assist the Care Manager in the day-to-day management of care services.
  • Develop and review individual care plans, ensuring they reflect the needs and preferences of clients.
  • Coordinate and monitor the delivery of care services, ensuring they meet regulatory and quality standards.
  • Supervise and support a team of care staff, providing guidance, training, and performance management.
  • Conduct client assessments and ensure appropriate care is provided in line with personalised care plans.
  • Act as a point of contact for families, external agencies, and healthcare professionals.
  • Ensure effective communication within the team and with other relevant parties.
  • Maintain accurate and up-to-date client records and documentation in line with company policies and procedures.
  • Promote a positive and supportive environment for clients and staff.
Skills and Experience
  • Previous experience in a care coordination or deputy care management role, or a similar field.
  • Strong understanding of person-centred care principles and regulatory standards.
  • Excellent communication skills, both written and verbal.
  • Experience in managing and supporting a team of carers.
  • Good organisational and time-management skills with the ability to prioritise tasks effectively.
  • A compassionate and empathetic approach to working with clients.
  • NVQ Level 3 in Health and Social Care (or equivalent) is desirable; Level 5 is a bonus.
