Band 6 Care Coordinator

TRUST: Pennine Care NHS

LOCATION - Tameside & Glossop Early Intervention Team base, 225 Old St, Ashton-under-Lyne OL6 7SR - plenty of parking.

HOURS - Part time, 2 days (between 9am - 5pm Mon - Fri) flexible days and would consider more / less hours if required.

MAIN DUTIES

To provide care coordination (case management) to an identified group of patients open the team- patients identified as being in the early phase of a psychotic illness. This will include; the assessment of needs and delivery of interventions in line with currently identified best practice; risk assessment and management and safeguarding where appropriate. You will be supported by the team manager and wider team.

Experience working with clients with serious mental illness together with knowledge and skills in assessment, planning, implementation and evaluation of the care of clients with serious mental illness.

Use of a car or access to a means of mobility and ability to travel is essential to the post.
