A leading medical centre in Tuxford is seeking a Care Coordinator to enhance their non-clinical team. The successful candidate will manage patient communications, assist with back office tasks, and play a vital role in patient care navigation. Ideal applicants will possess strong IT skills and previous experience in a GP practice.
Tuxford Medical Centre is seeking to appoint an additional member of staff to compliment their existing non-clinical team.
The ideal candidate will be able to work in a fast paced environment and be able to adapt to change easily. You will need to have a strong work ethic and be able to work to tight deadlines. The role involves patient contact both in person and on the phone, so good communication skills are required.
Our Care Coordinators provide extra time, capacity andexpertise to support patients in preparing for (or in following-up) clinicalconversations they have with primary care professionals. They will work closelywith the GPs and other primary care professionals to manage patients, makingsure that appropriate support is made available to them and their carers, andensuring that their changing needs are addressed.
Care Navigators ensure that the Practice runssmoothly and assist with other back office administration. This role provides ahigh level of service to patients, doctors, nurses and other members of thePractice team.
Although outwardly we may appear to be a "stereotypical small rural dispensing practice" - first impressions can be deceiving! Our premises were extensively renovated in 2019 and now provide a first class medical environment for staff and patients alike.

We foster team working across the practice, from GPs that have an 'open door' philosophy for staff queries, to our practice nurses having dedicated admin and nurse meeting time.
Job Summary:
To ensure that the reception area of the Practice runs smoothly and to assist with back office administration of medical records. This role provides a high level of service to patients, doctors, nurses and other members of the Practice team.

Telephone Duties
Reception Duties
Administrative Duties
Culling and sorting patient records and letters in preparation for summarising
Reviewing medical records and producing an accurate summary of the patients medical history
Read coding the information into the practice clinical system
o Auditing data collection standards in the practice
o Monitoring progress of notes summarising against practice targets
o Monitoring patient call and recall systems
Pulling Lloyd George envelopes when requested by doctors
Other
General housekeeping (e.g. keeping reception and waiting areas tidy)
Make refreshments for doctors and other team members, participate in washing up
Any other reasonable duties as necessary
