Location: Sudbury, Suffolk
Salary: £28,000 per annum (rising to £30,000 upon successful completion of probation)
Hours: Full-time, 40 hours per week (Days)
On-Call Responsibility: Included

We're looking for a dedicated and passionate Care Coordinator who is eager to make a real difference in the community. This is an exciting opportunity for someone ready to help shape a growing service while delivering exceptional, person-centred care to clients across the Sudbury area.
As a Care Coordinator, you'll be a key part of the care team, working both in the community and from the office (as the branch develops). You will be the first point of contact for both clients and carers - a hands-on and supportive presence in the lives of those we care for. You'll work closely with the office team and local health services to help make sure care is well organised, safe, and kind.
